LHH is currently recruiting for a seasoned and committed Case Manager in Los Angeles, CA 90021. This position demands exceptional leadership abilities, comprehensive knowledge of homelessness services, and the capability to manage diverse teams and intricate programs.
Key Responsibilities:
- Direct service delivery and crisis management for supportive housing programs.
- Collaborate with property management and community partners to facilitate smooth client transitions into permanent housing.
- Develop and implement policies, procedures, and quality assurance systems to ensure high-quality service delivery.
- Monitor program performance, funder reporting, and data compliance using HMIS/CHAMP systems.
Qualifications:
- Extensive knowledge of LA County homelessness services landscape and relevant policies.
- Experience working with diverse populations, including individuals with chronic physical and behavioral health issues.
- Proficiency in Microsoft Office and familiarity with HMIS/CHAMP systems is preferred.
- Strong written and verbal communication skills.
- Valid CA driver’s license, insurance, and reliable transportation.
- Bilingual skills are an advantage.
